Hospice Nursing Specialist

Job SummaryWe are seeking a Registered Nurse (RN) to join our team as a Hospice RN. In this role, you will provide compassionate care to patients in their homes or other settings, working closely with an interdisciplinary team to develop and implement individualized care plans.
Key Responsibilities:
- Provide direct patient care services to hospice patients and supportive care to the patient's family
- Act as primary liaison between physician, patient/family, and hospice team
- Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
- Maintain all assigned patient clinical records
- Participate in company quality assessment and performance improvement programs
Requirements:
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
- Current driver's license and automobile insurance
- Current CPR certification
Compensation:
We offer a competitive salary of $60,000 - $80,000 per year, depending on experience, as well as opportunities for career growth and professional development.
