The MDS Nurse utilizes independent judgment in the planning, organizing, directing, and evaluation of the activities of the professional and supportive nursing staff involved in the resident care plan.

This role involves evaluating the care provided to each resident and ensuring that care plans are up-to-date. The MDS Nurse leads the Resident Assessment Instrument (RAI) process and assists in the assessment and evaluation of potential admissions. Upon admission, as well as quarterly and annually, the MDS Nurse conducts resident assessments.

Collaboration is key, as the MDS Nurse works with residents, families, advocates, and interdisciplinary colleagues, including providers, to ensure ongoing care that promotes the highest quality of life for each resident.

When functioning on the floor, the Registered Nurse (RN) employs the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to deliver individualized nursing care to patients.

The RN demonstrates proficiency and practices within the full scope of nursing expertise, adhering to appropriate age and population-specific standards as designated in their clinical setting.

The RN is responsible for resident care across all phases of preventative care, health maintenance, diagnosis, and treatment. This includes coordination of care, resident assessment, education, and various nursing interventions.


As a charge nurse, the RN delegates appropriate tasks to licensed and unlicensed assistive personnel, ensuring that these tasks align with job descriptions, skills, resident care needs, and the competence level of the delegate.

In this capacity, the RN provides adequate support to delegates to ensure safe and reliable administration of resident care and effective resolution of care and service concerns.


The RN operates within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure.

The Professional Nursing Practice acknowledges the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as established by the American Nurses Association, serving as the foundation for nursing care delivery and professional conduct.

Qualifications


A graduate from a National League of Nursing (NLN) or American Association of Colleges of Nursing (AACN) accredited nursing program is preferred.

A Bachelor's Degree in nursing is preferred.

A minimum of one year of experience working as a licensed nurse with the senior population is preferred. Specific services or positions may have additional education, training, or experience requirements. Resident Assessment Coordinator-Certified (RAC-CT) and management or supervisory experience are preferred.


Currently holds an unencumbered RN license with the State Board of Nursing where nursing practice occurs and/or possesses multistate licensure if in a Nurse Licensure Compact (NLC) state.

Maintains required certifications and completes necessary continuing education units (CEU) and competency training. Affiliation with an external professional nursing organization is preferred.
