Position Overview

In this role, you will be responsible for educating healthcare professionals about hospice services and delivering end-of-life solutions that prioritize clinical excellence, compassion, and dignity.

Key Responsibilities:
  • Ensure effective and timely intake and referral processes to meet the needs of patients, families, and referral sources.
  • Complete admissions for referred patients in urgent situations, as staffing allows.
  • Provide accurate and timely information about hospice services to inquiries.
  • Respond promptly to calls from referral sources and initiate necessary paperwork for the intake process.
  • Act as a patient advocate in evaluating and developing care plans and obtaining informed consent for hospice services.
  • Engage with various account types, including Skilled Nursing Facilities, Assisted Living Facilities, Hospitals, and Physicians.
  • Conduct educational sessions on the benefits of hospice care.

Qualifications:

The ideal candidate will possess a compassionate heart, demonstrating kindness and empathy towards clients, their patients, and families.

  • Active RN license in the relevant state(s).
  • A minimum of two years of healthcare experience is preferred in settings such as hospice, home health, clinical liaison, or related fields.
  • Recognized and respected within the community.
