Overview: As a Hospice RN Case Manager, you will report to the Administrator, Executive Director, or Patient Care Manager Senior and provide direct patient care services, act as a primary liaison between physician, patient/family, and hospice team, manage an interdisciplinary group, evaluate patient and family needs, and maintain all assigned patient clinical records.
- Hospice heart: compassionate, kind, and empathetic to patients and families
- Demonstrate flexibility and composure in times of crisis
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
- Current driver's license and automobile insurance
- Current CPR certification
