Oswego County Opportunities is a leading provider of services for individuals with developmental disabilities. We are seeking a skilled and compassionate DSP Day/Community Habilitation Specialist to join our team.
Job Summary:
The DSP Day/Community Habilitation Specialist will work independently with assigned individuals or small groups to provide direct services that enhance the quality of life and promote community integration and skill development for greater independence.
Key Responsibilities:
- Provide person-centered support to individuals and adhere to OPWDD core competency expectations for a DSP.
- Follow individual protective oversight plans and safeguards to ensure safety and protection to individuals, respecting individual rights.
- Assist with daily living routines, including hygiene practices, toileting, and grooming, and preparing individuals for daily activities in the community.
- Facilitate person-centered plans for individuals during Day Hab/Community Hab shifts.
- Assist with medical needs, provide first-aid treatment as necessary, and assist with meeting individuals' dietary needs.
- Perform counseling and coaching of individuals, analyze situations, and use good judgment and proper intervention techniques; follow behavior management plans.
- Participate in and implement habilitation plans; ensure provision of Medicaid billable services with accurate documentation and billing records using Precision Care software; report concerns to supervisor.
- Ensure proper handling of Agency money and documentation for approved purchases, assist with individuals' personal allowance purchases, ensuring correct accounting and balances with documentation according to Agency and program procedures.
- Schedule and participate in activities, ensure community integration, and provide transportation; use agency van when available or personal vehicle if needed.
- Document and report incidents in accordance with OPWDD regulations and Agency procedures.
- Follow Agency policy and procedures in emergency and non-emergency situations, handle situations that arise, and seek assistance on unusual situations and report concerns to supervisor or on-call manager.
- Reports concerns that arise during assigned shift to supervisor.
- Maintain paperwork as required by state and program regulations and Agency policy in an accurate and timely manner with attention to detail.
- Maintain confidentiality of individuals, program, and site-related information.
- Assist with snow removal as needed and assigned.
- Provide coverage on extended shifts until coverage is secured in urgent situations.
Requirements:
- Ability to work assigned schedule and be reasonably flexible, may need to work an extended shift in urgent situations.
- Ability to relate well to developmentally disabled populations and their families.
- Ability to exercise good judgment in carrying out duties, follow regulations, and defuse behavioral concerns.
- Ability to work with others in a warm, professional manner and be a positive role model to individuals.
- Valid NYS Driver's license with good driving record in accordance with Agency policy/procedure.
- Access to a reliable, properly registered, inspected, and insured vehicle for related area travel/transport.
- Becoming certified Agency Vehicles which may include driving 12–15 passenger vans.
- Becoming and maintaining 19-A driving certification.
- Acceptable physical ability to carry out the position, including the ability to perform within the DD program Functional Job Description, including but not limited to: lifting regularly, pushing wheelchairs, assisting in transferring individuals, and climbing stairs.
