Occupational Therapist PRN-OT Home Health

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Occupational Therapist to join our team of skilled home healthcare providers. As an Occupational Therapist, you will play a vital role in helping our patients achieve their goals and regain independence in their daily lives.
Responsibilities:- Evaluate patients' functional status and develop personalized treatment plans to improve their ability to perform daily activities.
- Conduct home assessments to identify potential hazards and barriers to independent living, and provide recommendations for equipment and adaptive devices.
- Develop and implement treatment programs to improve patients' physical and cognitive abilities, and communicate progress to referring physicians and other healthcare professionals.
- Teach patients and their caregivers new skills and techniques to promote independence and safety in the home environment.
- Maintain accurate and detailed clinical records, and submit reports to physicians as required.
- Collaborate with other healthcare professionals to ensure coordinated and comprehensive care for our patients.
- Master's degree in Occupational Therapy from an accredited program.
- Current licensure as an Occupational Therapist in the State of Florida.
- Two years of clinical experience as an Occupational Therapist.
- Current BLS certification.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
