Key responsibilities of the role:

The Palliative Care Nurse Coordinator (RN) is an integral member of the clinical team, leading by example. The Hospice RN is tasked with delivering professional nursing care to patients in alignment with the established care plan. The Hospice RN ensures high-quality care during patient interactions and while completing all necessary documentation. Compliance with agency policies and procedures is paramount. The RN will provide care that meets all relevant professional standards and adheres to state and federal regulations.

  • Conducts thorough subjective and objective assessments of patient status, encompassing physical, psychosocial, and environmental factors.
  • Ensures adherence to federal and state regulations governing hospice services by following established policies and procedures.
  • Develops individualized care plans based on assessment data, modifying and updating them at least every two weeks for assigned patients, as needed.
  • Implements skilled interventions aimed at achieving realistic outcomes within specified timeframes.
  • Performs interventions within the nursing scope of practice.
  • Maintains accurate and comprehensive documentation of all clinical data, including admissions, physician orders, nursing visits, and changes in patient conditions.
  • Demonstrates proficiency in technical nursing skills in accordance with legal scope of practice.
  • Provides education tailored to patient and family needs, evaluating and adjusting as necessary.
  • Conducts clinical chart reviews as requested.
  • Handles on-call responsibilities as assigned.
  • Efficiently performs patient visits, ensuring timely completion and submission of all documentation per branch policies.
  • Assesses patient eligibility for admission and assists in assigning qualified personnel.
  • Collaborates with physicians to develop and revise care plans based on patient conditions and needs.
  • Prepares clinical progress notes and continuously reviews documentation to ensure compliance with hospice standards.
  • Ensures that adequate medical supplies and equipment are provided and reordered as necessary for assigned patients.
  • Adheres to organizational patient care policies, including implementation of patient care protocols.
  • Conducts supervisory and educational visits for the evaluation of professional and paraprofessional staff on assigned cases, providing follow-up information to management as requested.
  • Offers orientation, training, or in-service sessions to the care team to ensure quality and appropriateness of care provided, and precepts new staff as required.
  • Ensures that documentation by Home Health Aides is complete and appropriate to the services rendered.
  • Follows state licensure requirements and Intrepid policies by adhering to agency protocols.
  • Recommends new policies or modifications to existing policies to enhance patient service quality and operational efficiency.
  • Completes additional assignments and duties as requested to achieve the company’s goals and objectives.
