About the Role

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health RN. As a key member of our healthcare team, you will provide high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Provide comprehensive home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases.
  • Work collaboratively with a multidisciplinary team of healthcare professionals, including physicians, LPNs, CNAs, and aides, to ensure seamless patient care.
  • Conduct thorough patient assessments, document progress, and report changes to the patient's physician.
  • Develop and implement personalized care plans, educating patients on their treatment, medication, therapy, diet, and exercise.
  • Coach family members on patient care, medication, and home safety to ensure a smooth transition.
  • Ensure patient goals are met and coordinate discharge from services.
Requirements
  • Active Registered Nurse (RN) License in Colorado.
  • Graduate of an accredited school or program of nursing.
  • At least 1200 hours of experience as an RN.
  • Healthcare CPR/BLS Certification (demonstration course required).
  • Home health care experience preferred.
  • Pass a background check and drug test (we are required to test for THC).
