About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Home Health Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with an interdisciplinary team to deliver whole-person care.

Responsibilities
  • Provide direct patient care, including assessments, interventions, and education
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
  • Communicate effectively with patients, families, and caregivers to ensure understanding and satisfaction
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences
Requirements
  • Graduate of an accredited school of nursing
  • Current Washington Registered Nurse License
  • National Provider BLS - American Heart Association certification
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • 2 years of full-time experience practicing as a registered nurse in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing Facility
  • 1 year of recent, hands-on technical clinical experience in a Medicare-certified Homecare or Hospice agency/facility
  • Experience with wound care, venipuncture, ostomy, catheters, and feeding tubes
  • Rehab nursing experience
  • Experience working with an EMR system
