Job Summary: We are seeking a highly skilled and experienced Program Coordinator II to join our team at The Lundquist Institute. The successful candidate will be responsible for coordinating and facilitating services delivered by the Medical-Financial Partnership Program in prenatal, perinatal, pediatric, and family medicine care settings. Key Responsibilities: * Coordinate and supervise Medical-Financial Partnership team activities to ensure continuity of services and optimize service delivery. * Develop and implement a life course, two-generation resilience-building intervention in clinical settings. * Provide direct clinical support for the Medical-Financial Partnership interdisciplinary program to promote trust-based parent-provider relationships and parenting capacity of parents. * Collaborate with interdisciplinary teams to promote prenatal, perinatal, and pediatric secure attachments and early relational health. * Assist in the collection of relevant clinical measures for patient care and program support. * Perform professional social work services to patients and families with complex medical and related psychosocial needs. * Provide ongoing education and supervision of social work interns/resilience coaches. * Participate in multi-disciplinary team conferences to discuss and evaluate client progress and develop care coordination and care transition plans. Requirements: * Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education. * Two years of clinical social work experience. * Valid and active license as a Licensed Clinical Social Worker issued by the California Department of Consumer Affairs, Board of Behavioral Sciences. * Ability to work in a fast-paced environment with minimum supervision. * Spanish language fluency. Salary Range: Up to $108,000/yr.
