Role Summary:

The Lead Case Management Specialist will play a crucial role in supporting the Program Manager to ensure the smooth operation of the Preferred Communities Program. This position focuses on delivering comprehensive case management services to clients with unique health requirements. The Specialist will work closely with the Program Manager to uphold compliance with funding agency standards, organizational policies, and departmental guidelines.

Key Responsibilities:

  • Collaborate effectively with the Program Manager
  • Participate in partnership meetings and community outreach events
  • Take the lead on designated projects
  • Ensure adherence to all relevant laws and regulations
  • Assist the Program Manager in organizing case documentation
  • Provide training and mentorship to team members
  • Coordinate transportation and accompany clients to necessary appointments
  • Support clients in fulfilling service provider expectations
  • Conduct home visits and liaise with referral agencies
  • Document significant client behaviors or incidents
  • Identify and secure resources to address individual client needs
  • Maintain accurate records and engage in continuous professional development

Qualifications:

  • Education: Bachelor's degree in a relevant field preferred
  • Credentials: Valid NYS Driver's license required
  • Experience: 1-2 years in the Human Services sector

Work Environment:

The role involves various physical activities including sitting, standing, walking, and light lifting, along with specific visual capabilities. The work environment typically maintains a moderate noise level.
