Job Summary

We are seeking a skilled and compassionate Clinical Care Nurse to join our team at Greater Lynn Senior Services. As a key member of our LTSS Program, you will play a vital role in providing high-quality care to our clients.

Key Responsibilities
  • Conduct comprehensive assessments, intakes, and care plans to ensure the best possible outcomes for our clients.
  • Provide oversight to enrollees' discharge plans from the hospital or ED, and monitor ADT notifications for care transitions support.
  • Support the Program Director in day-to-day operations of the program, including team meetings and training as needed.
  • Participate in all program evaluation and quality improvement activities related to EOHHS' LTSS Community Partnership program and other NRLP-related activities.
Requirements
  • Current MA RN License
  • Three (3) years of clinical experience
  • Three (3) years' experience in community nursing
  • Experience providing care management in a health and/or human services organization
  • Previous work experience working with individuals with disabilities
  • The ability to handle confidential and personal information
  • Proficient computer skills
  • Strong writing and communication skills
  • Reliable transportation and a valid driver's license to make visits throughout the GLSS service area
