About the Role
CareStar is seeking a dedicated Intensive Support Coordinator to join our Clinical department. In this role, you will identify, coordinate, and oversee the delivery of services to enhance the health, safety, and general wellbeing of participants, assisting them in achieving maximum independence.
Responsibilities
- Develop and implement personalized service plans to meet the unique needs of participants.
- Collaborate with healthcare professionals, family members, and caregivers to ensure comprehensive support.
- Conduct regular assessments and evaluations to monitor progress and make adjustments as needed.
- Provide education and training to participants and their support networks on health and wellness strategies.
- Ensure compliance with CareStar policies and procedures, maintaining confidentiality and adhering to the CareStar Rule.
Requirements
- Bachelor's degree in a Human Service field, such as Social Work, Psychology, or Sociology.
- Two years of direct service experience serving individuals with disabilities or related disabilities.
- Experience in healthcare, habilitative, or rehabilitative settings, with a focus on care coordination and management.
- Effective organizational and communication skills, with the ability to work independently and as part of a team.
- Valid driver's license, reliable transportation, and car insurance, as required by state law.
