Requirements

  • Bachelor's degree in a Human Service field, such as Social Work, Psychology, or Sociology.
  • Two years of direct service experience serving individuals with disabilities or related disabilities.
  • Experience in healthcare, habilitative, or rehabilitative settings, with a focus on care coordination and management.
  • Effective organizational and communication skills, with the ability to work independently and as part of a team.
  • Valid driver's license, reliable transportation, and car insurance, as required by state law.
