Job Summary:

UPMC is seeking a skilled Care Manager to join our team. As a key member of our care management department, you will play a critical role in coordinating the clinical and financial plan for patients. Your expertise will ensure that patients receive the necessary care and support to achieve optimal outcomes.

Responsibilities:

Collaborate with patients, caregivers, and healthcare providers to develop and implement individualized care plans.
Conduct thorough assessments to identify patient needs and develop strategies to address them.
Work closely with the multidisciplinary team to ensure seamless care coordination and communication.
Utilize InterQual criteria to determine the most appropriate level of care for patients.
Obtain necessary authorizations and approvals for post-acute care services.

Requirements:

Graduate of an approved school of nursing.
Two years of nursing experience, with a focus on care management or a related field.
BSN or related bachelor's degree required.
Current licensure as a professional nurse in the Commonwealth of PA.
