Key Responsibilities- Assist the Registered Nurse (RN) in carrying out the patient's plan of care, ensuring seamless and effective care delivery.
- Perform specialized procedures as directed by the RN, maintaining the highest standards of patient safety and well-being.
- Collaborate with the RN to prepare, implement, and evaluate the patient's plan of care, ensuring alignment with agency policies and procedures.
- Reinforce teaching of end-of-life care to patients and their families, promoting understanding and empowerment.
- Participate actively in staff and team conferences, IDG meetings, and other relevant discussions to ensure comprehensive care coordination.
- Prepare clinical progress notes as necessary, maintaining accurate and detailed records of patient care.
- Notify the RN of any changes or concerns noted during patient visits, ensuring prompt attention and resolution.
- Perform on-call duties, including holidays, as per agency policy and need, demonstrating flexibility and commitment to patient care.
- Graduation from an accredited School of Vocational Nursing.
- Licensed in California as an LVN.
- At least one year of experience as an LVN, with preference given to those with hospice experience.
- Licensed driver with an insured automobile in good working order.
- Excellent communication skills, both verbal and written.
- Proven track record of dependability and flexibility.
