Job Title: LPN LVN Caregiver

Job Summary:
Providence Service is seeking a skilled LPN LVN Caregiver to join our team. As a Licensed Practical Nurse, you will work alongside an interdisciplinary team to provide whole-person quality care to patients in need. Our team is dedicated to delivering exceptional patient care and making a positive impact in our community.

Responsibilities:
Provide direct patient care, including administering medications, wound care, and vital sign monitoring
Collaborate with the healthcare team to develop and implement individualized care plans
Educate patients and their families on disease management, medication adherence, and healthy lifestyle choices
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:
Current California Licensed Vocational Nurse (LVN) license
National Provider BLS - American Heart Association certification
1 year of experience in nursing, hospice, or related health field
