Clinical Pharmacist

The primary function of the Clinical Pharmacist is to guarantee the safe and effective utilization of medications, enhance the pharmaceutical care provided to patients, and offer expert drug information to fellow healthcare professionals and patients alike. The Clinical Pharmacist also oversees the efficient preparation and distribution of pharmaceuticals, while providing direct supervision to Pharmacy Technicians within the facility.

Key Responsibilities
- Analyzes, reviews, and assesses medication orders to determine optimal dosing, dosage forms, frequency, concentration, duration, and potential interactions, considering the unique needs of patients to ensure high-quality pharmaceutical care and accurate medication dispensing.
- Dispenses medications by verifying patient-specific bulk medications and ensuring the accuracy of pyxis fills and refills, confirming the correct medication, dosage, form, strength, size, and quantity for safe and timely pharmaceutical care.
- Engages in process improvement initiatives within the department and organization to foster strong relationships with colleagues, embody organizational values, and enhance financial sustainability.
Education and Experience:
- Bachelor of Science in Pharmacy or Doctor of Pharmacy from an ACPE-accredited pharmacy program.
- Preferred experience in a hospital pharmacy setting.
- Active license to practice pharmacy in New York State or eligibility for licensure reciprocity from another state.
- Pharmacy Registration in New York State.
- Strong communication skills to interpret medication orders and respond effectively to urgent situations, inquiries, and complaints from staff, physicians, and patients.
- Ability to perform complex pharmaceutical calculations and understand statistical concepts.
- Capability to resolve clinical issues related to pharmaceutical care and medication therapy.
- In-depth knowledge of current pharmacy practices and principles.
- Proficiency in computer order entry and understanding of federal and state pharmacy regulations.
- Ability to work independently within established policies and achieve program goals.
- Demonstrated judgment, tact, and diplomacy in professional interactions.
