Job Summary

We are seeking a highly skilled and compassionate Recovery Navigator to join our Hub and Spoke Health Home team. As a Recovery Navigator, you will work collaboratively with individuals to develop comprehensive care plans, facilitate monthly recovery monitoring meetings, and ensure seamless access to quality care services.

Key Responsibilities
  • Engage individuals in care and services through a strength-based, trauma-informed approach
  • Closely collaborate with the Lead Recovery Navigator and WCS Hub Team to serve individuals referred to the program
  • Provide direct service to approximately 12 individuals weekly
  • Complete a Recovery Life Story Tool and Assessment (RLST/PRP) in the person's own words
  • Facilitate monthly BAM monitoring/team meetings in collaboration with the person's care team
  • Coordinate care with providers of services and verify services are provided
  • Work closely with the person to develop a team of informal and formal supports
  • Provide transportation and attend appointments with the person
  • Support crisis response and develop strategies to meet immediate needs
Requirements
  • High school or GED/HSED required; Bachelor's degree in Social Work, Psychology, Nursing, or Social Services preferred
  • Knowledge and experience serving people with substance use needs
  • Working knowledge of computer applications preferred (Windows and Microsoft Word)
  • Ability to display cultural competence and respond respectfully to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors
  • Valid driver's license, automobile, and insurance sufficient to meet agency requirements
