The primary focus of this role is to oversee and manage all facets of rehabilitation services within the designated communities while also maintaining a clinical caseload.

The Rehabilitation Services Manager will be responsible for understanding and addressing the needs of clients and partners, fostering an atmosphere that upholds exceptional clinical and financial integrity. This role acts as an advocate, educator, liaison, and mentor to a multidisciplinary team, including community staff, home health personnel, hospice workers, physicians, nurses, and others.

Key Responsibilities


• Maintain a clinical caseload.


• Exhibit strong clinical expertise and comprehend the role of rehabilitation across various clinical disciplines and service lines within Assisted Living, Independent Living, and Continuing Care Retirement Communities (CCRC).


• Stay informed about Federal, State, and Local regulations regarding therapy service delivery in Assisted Living, Independent Living, and CCRC environments, ensuring compliance among all staff.


• Apply knowledge of life span growth and development principles to assess resident data and provide age-appropriate care according to departmental policies.


• Demonstrate effective planning, organizational, and interpersonal skills to maintain a safe and goal-oriented work environment for oneself and the therapy team.


• Represent HealthPRO Heritage professionally, addressing community customer concerns and resolving issues effectively.


• Serve as a liaison between management, Home Health, Hospice, and community administration, communicating clearly and diplomatically.


• Integrate clinical programs while addressing community-specific marketing needs.


• Participate in community-based committees to promote collaborative goals for clinical programs and marketing while ensuring responsible time management.


• Engage in Quality Improvement Programs.


• Attend patient care meetings to facilitate open communication while adhering to HIPAA guidelines.


• Provide educational sessions to the community, healthcare providers, and staff regarding therapy and facility services.


• Participate in community events to promote therapy services.


• Plan, schedule, coordinate, and oversee rehabilitation services for the department, including vacation approvals.


• Assess staffing needs and create schedules to meet resident care demands, assisting with recruitment as necessary.


• Foster positive employee relations through open communication and problem-solving during scheduled team meetings.


• Conduct performance evaluations for rehabilitation personnel, collaborating with supervisory staff to review performance and set goals.


• Oversee documentation audits to ensure timely and accurate clinical documentation, submitting reports to supervisory personnel.


• Monitor personnel compliance with credentialing standards and improvement plans as needed.


• Investigate and assist in resolving staff, service, and resident-related issues as required.


• Develop special programs for resident engagement, such as fitness and dementia programming.


• Provide leadership and direct management to clinical therapy staff, fostering a collaborative team environment.


• Uphold the ethical standards of the organization and report non-compliance to supervisory personnel.


• Exhibit a willingness to work with residents who may be physically, emotionally, or cognitively compromised.


• Be prepared to work in environments with potential risks, including exposure to infections and hazardous materials.


• Manage the emotional stress associated with working with residents, families, and colleagues.


• Work independently with minimal supervision and demonstrate basic computer skills.


• Supervise Rehabilitation Technicians, students, and volunteers as needed.


• Evaluate the quality and effectiveness of therapy services and individual programs based on established criteria.


• Deliver treatment in accordance with established standards of practice and company guidelines.


• Exhibit sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.


• Arrive on time and coordinate schedules to meet performance standards.


• Ensure a safe working environment and comply with infection control and safety standards.


• Report hazardous conditions and incidents to appropriate personnel immediately.


• Respond appropriately in emergency situations.


• Maintain high standards of cleanliness, grooming, and professional attire.


• Contribute positively to the team by sharing information and accepting constructive feedback.


• Adhere to all company policies, including job descriptions and confidentiality standards.


• Adapt positively to changes in responsibilities and be open to new methods of task completion.


• Engage in open communication with supervisors.


• Interact professionally and respectfully with rehabilitation staff, community staff, residents, and families.


• Follow End of Month policies for accurate claim submissions.


• Prepare and submit reports in a timely manner.


• Analyze and interpret budget expectations consistently.


• Perform additional duties as assigned to effectively fulfill the responsibilities of the position.


• Occasional overnight travel may be required.

Qualifications


• Bachelor’s or Master’s Degree in a relevant field.


• Commitment to ongoing education and professional development.


• Valid state professional licensure is required.
