Job Summary

The Speech/Language Pathologist will provide high-quality evaluation, treatment, and supervision services to patients with communication, cognitive, and swallowing difficulties. This role requires strong interpersonal skills, emotional intelligence, and the ability to exercise independent judgment and discretion.

Key Responsibilities
  • Evaluate patients for rehabilitation services within 24 to 48 hours of physician's orders.
  • Develop effective treatment plans that address patient needs and goals.
  • Treat patients according to the treatment plan of care, ensuring accurate and timely documentation.
  • Communicate with healthcare team members, patients, and families to ensure seamless care coordination.
  • Participate in care conferences and interdisciplinary team meetings to share knowledge and best practices.
  • Provide in-service training to facility staff on restorative and functional maintenance programs.
  • Record evaluations, treatment notes, progress notes, and discharge summaries in a timely and accurate manner.
  • Meet or exceed minimum productivity standards established by Concept Rehab.
  • Recommend equipment and resources to support improved clinical services.
  • Act as a clinical supervisor for affiliating Speech/Language Pathology graduate clinicians.
  • Comply with state-specific Speech Pathology licensure standards.
Requirements
  • Master's Degree in Speech-Language Pathology.
  • Certification by the American Speech-Language-Hearing Association.
  • Licensure in the state(s) where services are provided to practice as a Speech/Language Pathologist.
