Role Summary:
The Rehabilitation Therapist is responsible for assessing, designing, and executing individualized care plans aimed at enhancing, preserving, or restoring functionality, while minimizing or preventing disabilities. The ultimate goal is to assist patients in achieving their highest potential.

Key Responsibilities:

  • Interpreting medical prescriptions, conducting initial assessments, and formulating treatment plans with specific functional or occupational objectives.
  • Progressively advancing treatment strategies and goals.
  • Maintaining timely communication with referring physicians.
  • Managing discharge processes and ensuring patient follow-up.
  • Overseeing rehabilitation aides and administrative personnel as necessary.
  • Supervising Physical Therapist Assistants in accordance with state regulations.
  • Documenting all treatment activities and communications promptly, including initial evaluations, progress notes, discharge summaries, daily S.O.A.P. notes, and insurance documentation.
  • Building and nurturing professional relationships with referral sources, including physicians, physician assistants, nurse practitioners, chiropractors, and case managers.

Expectations:

  • Delivering outstanding patient care and service.
  • Engaging in community outreach initiatives, such as:
    • Participating in health fairs.
    • Conducting educational presentations on orthopedic injuries or prevention.
    • Providing coverage for special events like sports camps and races.
    • Offering complimentary public screenings with appropriate referrals.
  • Exhibiting compassion and a service-oriented mindset towards patients.
  • Communicating effectively with physicians about our services and commitment to quality care.
  • Facilitating patient visits to their healthcare providers.
  • Participating in interoffice communication and follow-up.
  • Engaging in ongoing education and training, including staff in-services and community presentations.

Additional Responsibilities:

  • Maintaining a valid state license.
  • Meeting all compliance standards as required.
  • Demonstrating enthusiasm and teamwork.

Qualifications:
The following qualifications are essential for success in this role:

  • Degree from an accredited Physical Therapy program.
  • Current professional licensure as a Physical Therapist.
  • Valid CPR certification.

Physical Requirements:
