Medical Case Worker

Healthcare Staffing Professionals, Inc. is seeking a highly skilled Medical Case Worker to join our team. As a Medical Case Worker, you will be responsible for providing comprehensive assessments, developing individualized care plans, and coordinating services and supports for patients and clients in hospitals, clinics, rehabilitation, residential, or healthcare facilities.
Key Responsibilities:- Conduct comprehensive assessments of patients' medical, social, and psychological needs.
- Develop and implement individualized care plans in collaboration with patients, healthcare teams, and other relevant parties.
- Provide case management services, including coordination of services and supports, to ensure timely access to necessary medical services.
- Maintain accurate and up-to-date documentation of client assessments, care plans, interventions, and progress.
- Collaborate with healthcare professionals, social workers, and other stakeholders to ensure effective communication and coordination of care.
- One year of experience providing services to patients and clients in hospitals, clinics, rehabilitation, residential, or healthcare facilities.
- Bachelor's degree from an accredited college or university and one year of professional casework experience interviewing, counseling, and assisting patients/clients with social problems.
- A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
- Homeless health care experience.
- Experience with delivering clinical care services within ARFs and RCFEs.
- Familiarity with complex care management services for the care of high-need/high-cost Medical beneficiaries.
- Ability to lead teams in performance improvement activities.
- Demonstrated teaching and communication skills with non-clinical and clinical staff.
