Position Overview:
As a Case Manager, you will play a crucial role in supporting children, adolescents, and teens with social, emotional, and behavioral difficulties. Your responsibilities will include:
- Coordinating the creation, assessment, and execution of personalized service plans and behavior intervention strategies.
- Monitoring the quality of care provided to individuals and their families, ensuring adherence to contractual obligations and regulatory standards.
- Building and nurturing relationships with families and external case management professionals.
Qualifications:
To be successful in this role, you should possess:
- A Bachelor's degree in human services or a related discipline.
- A minimum of one year of relevant experience working with individuals with intellectual and developmental disabilities or at-risk youth.
- A valid driver's license, vehicle registration, and auto insurance.
- Required state licensure and training, including CPR, First Aid, and Behavioral Intervention Techniques.
- A reliable and compassionate demeanor, coupled with a commitment to delivering high-quality services.
