We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality patient care in the comfort of their own homes. As a Home Health LPN, you will be responsible for delivering exceptional clinical services, working closely with our interdisciplinary team to ensure the best possible outcomes for our patients.
Key Responsibilities- Provide high-quality clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care and in coordination with other healthcare team members.
- Continually observe and report patient nursing needs to supervising RNs.
- Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs, visit assignments, and schedule changes.
- Document patient visits thoroughly and completely, maintaining accurate and current medication listings.
- Participate in performance improvement initiatives to reduce hospitalizations and improve patient outcomes.
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
