Job Summary
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team as a PRN (as needed) member. In this role, you will provide high-quality patient care and technical skilled care to patients in their homes.
Key Responsibilities
- Provide quality patient care and technical skilled care to patients in their homes, following the plan of care and in coordination with other healthcare team members.
- Continuously observe and report patient nursing needs to supervising RNs.
- Communicate effectively with physicians, patients, family members, and agency staff regarding patient care issues or needs.
- Document patient visits thoroughly and completely, maintaining an accurate and current medication listing.
- Participate in performance improvement activities to reduce patient hospitalizations and improve outcomes.
Requirements
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
