Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Care team at AdventHealth. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver comprehensive care.

Responsibilities
  • Administer nursing care to patients in accordance with physician orders and under the direction of a Registered Nurse.
  • Provide individualized nursing care, teaching, and supervision to patients and their families or caregivers to ensure continuity of care.
  • Deliver high-quality nursing care, perform technical nursing skills with a high degree of proficiency, and promote a team approach in the delivery of patient care.
  • Execute physician orders, notify RN/Case Manager of necessary updates or changes in condition, and complete documentation according to Agency Policy and Regulatory guidelines.
  • Perform home visits as ordered and contribute to the development of the patient's plan of care.
Requirements
  • Vocational training in an Accredited School of Licensed Practical Nursing.
  • Minimum of 1 year of clinical experience in Medical/Surgical nursing.
  • Current LPN licensure in the state of Florida.
  • Current BLS certification.
  • Valid Driver's License.
Preferred Qualifications
  • Venipuncture skills preferred.
  • 2 years of Home Health nursing experience.
