Job Title: Pharmacist Clinical I-Inpatient

We are seeking a skilled and dedicated Pharmacist Clinical I-Inpatient to join our team at Kaiser Permanente in Honolulu, Hawaii.

About the Job:

  • Prepare and dispense medications using appropriate techniques and applicable policies and procedures.
  • Research and organize patient-specific information.
  • Document patient medication histories.
  • Recommend or review laboratory tests.
  • Analyze requestors drug information need.
  • Formulate response.
  • Follow up consistently on drug information recommendations and assess effectiveness.
  • Participate in monitoring and improving relevant outcome measures and/or quality improvement indicators.

Responsibilities:

  • Design and recommend pharmacotherapeutic regimens and corresponding monitoring plans.
  • Utilize patient interview/assessment skills, current literature-supported evidence and practices, drug initiative information, established protocols and integrates patient-specific disease and drug information, current practice standards, ethical issues, and quality-of-life issues, and pharmacoeconomic principles.
  • Monitor data and modifies pharmacotherapeutic plans as necessary.
  • Documents pharmaceutical care activities in patients medical records or clinical information system.
  • Provide effective medication-use education for patients and caregivers.
  • Provide one-on-one counseling to patients and caregivers.
  • Provide patient-specific verbal or written consultations, including pharmacotherapy and pharmacokinetic, to healthcare practitioners.

Qualifications:

  • Graduate from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE).
  • Pharmacist License (Hawaii) required at hire.
  • Demonstrated ability to safely handle and/or prepare various pharmaceutical compounds (e.g., chemotherapeutic agents, biologicals).
  • Demonstrated knowledge of and skill in word processing, spreadsheet and database PC applications.

Salary: $120,000 - $180,000 per year, depending on experience.
