**Job Summary**


We are seeking a highly skilled and motivated public health specialist to join our team. The ideal candidate will have a Master's degree in Social Work and a valid LMSW or LCSW license issued by the New York State Department of Education. The successful candidate will have experience in HIV prevention and treatment, as well as strong communication and interpersonal skills.


**Key Responsibilities**


The successful candidate will be responsible for:


1. Assisting ACE staff with complex case investigations and contact tracing.


2. Counseling HIV-diagnosed persons on HIV/AIDS test results, transmission risk, and partner notification.


3. Assisting HIV-diagnosed persons with disclosure of their HIV status to their partner(s) and providing education on HIV PrEP/PEP.


4. Assisting HIV-diagnosed persons with linkage to HIV clinical care.


5. Linking HIV-negative partners to PrEP providers for evaluation, counseling, and treatment.


6. Identifying barriers to care and connecting clients to supportive services, mental/behavioral health, and substance misuse resources.


7. Educating clients on ongoing access to HIV prevention and ancillary services.


8. Promoting knowledge of New York State HIV-related laws and regulations among New York City providers.


9. Identifying citywide and community resources, establishing, and maintaining linkages with community organizations and partners.


10. Collecting data on HIV cases to fulfill surveillance and case investigation data requirements.


11. Participating in ACE case management cohort review meetings and providing input to assist with case and contact investigation.


**What We Offer**


We offer a competitive salary of $ **$90,000 - $110,000** per year, depending on experience.
