About the Role

This 12-hour night shift position is an exciting opportunity to utilize your nursing expertise and provide patient-centered care.



  • Experience working in acute care facilities is preferred but not required.

As a Nursing Care Coordinator, you will be responsible for developing comprehensive care plans, implementing evidence-based practices, and collaborating with patients, families, and healthcare providers to ensure seamless care transitions.


Key Responsibilities

Your key responsibilities will include:



  • Collecting relevant data and analyzing assessment information to develop tailored care plans.
  • Collaborating with interdisciplinary teams to prioritize patient needs and optimize care outcomes.
  • Evaluating patient progress and making adjustments as needed to achieve optimal results.

You will also participate in discharge planning, delegate tasks to healthcare team members, and perform other job-related duties as assigned.


Requirements

To be successful in this role, you must possess:



  • A Bachelor's Degree in Nursing (BSN) or equivalent experience.
  • Active RN licensure and current CPR/BLS certification.
  • Crisis Prevention Intervention (CPI) or Techniques for Effective Aggression Management (TEAM) certification.

We offer a competitive salary range of $85,000 - $115,000 annually, depending on experience.
