Position Summary
In this role, you will deliver comprehensive case management services to clients participating in the YMCA Permanency Programs. This includes traditional and long-term foster care, extended foster care, therapeutic foster care, intensive in-home stabilization services, and outpatient mental health support. Your responsibilities will encompass coordinating or providing essential services that empower children and their caregivers to access medical, educational, social, clinical, and other necessary resources to maintain the child's overall health and development. Additionally, you will provide leadership and support to the broader Case Management team, ensuring that all clients in the Permanency Program receive high-quality case management services.

Key Responsibilities

  • Deliver case management and care coordination services as specified in relevant contracts and guidelines for clients enrolled in the Permanency Programs.
  • Maintain a manageable caseload in accordance with regulatory and contractual standards.
  • Ensure clients receive all necessary screenings, assessments, and services to address their comprehensive needs in a timely manner.
  • Collaborate with the Program Manager and child & family team to facilitate smooth transitions for clients, including placements and discharges.
  • Support foster parents and caregivers by providing necessary resources and services to meet client needs.
  • Document all services provided in compliance with regulatory requirements.
  • Conduct intake assessments and create individualized service plans for clients.
  • Participate in clinical consultations as needed to advocate for clients' mental health needs.
  • Perform additional duties as assigned.

Qualifications

  • Bachelor's degree in psychology, social work, or a related field; a Master's degree is preferred.
  • Experience working with youth in various supportive environments.
  • Strong communication and relationship-building skills.
  • Ability to adapt to diverse learning styles and plan supportive activities.
  • Basic computer proficiency and familiarity with Microsoft Office.
  • Valid driver's license and proof of insurance required for transportation of clients.
  • Current First Aid and CPR certification.
  • Knowledge of diverse populations and cultural competency is preferred.
