Job Summary:
This position provides respite care services to individuals enrolled in Self Direction. The successful candidate will work closely with the fiscal intermediary agency to manage daily activities, maintain accurate documentation, and ensure the safety and well-being of service recipients.

Responsibilities:
* Provide relief for in-home caregivers and support for individuals with developmental disabilities
* Complete and maintain required documentation for billing, including daily notes and data sheets
* Conduct risk assessments and home visits to ensure safe and healthy living environments
* Transport service recipients as required, using a reliable and properly insured vehicle

Requirements:
* High School Diploma or equivalency
* Valid New York State Driver's License
* Ability to lift up to 35 pounds and meet LAI Vehicle Operator Requirements
* Successful completion of training programs, including First Aid/CPR, SCIP, and Medication Administration

Estimated Salary Range:
$40,000 - $60,000 per year, depending on experience and qualifications.
