Job Overview

We are seeking a skilled Per Diem Home Health Nurse to join our team at CenterWell Home Health. As a key member of our care team, you will provide high-quality patient care and support in the comfort of patients' homes.

Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for home health patients, ensuring seamless care transitions.
  • Develop, implement, and evaluate individualized care plans that cater to each patient's unique needs and goals.
  • Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Conduct thorough assessments, document patient progress, and maintain accurate records.
  • Manage medication, treatments, and other medical interventions as prescribed by physicians.
  • Maintain open communication with patients, families, and caregivers to address concerns, answer questions, and provide education on self-care and disease management.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing from an accredited institution.
  • At least one year of experience in home health, med-surg, ICU, ER, or acute care settings.
  • Current RN licensure and CPR certification.
  • Excellent communication, organizational, and critical thinking skills.
