Care Management Program Manager

About the Role
We are seeking an experienced Care Management Program Manager to join our team. The successful candidate will be responsible for overseeing daily operations in our Partnership in Care Department. This role requires strong leadership skills, clinical expertise, and a passion for improving member outcomes. The salary for this position is estimated to be around $120,000 per year, based on industry standards and qualifications.
Responsibilities
This is a key leadership role that involves:
- Providing clinical guidance and supervision to all care management programs, based on accepted principles of nursing, social work, gerontology, geriatrics, HIV care, and care management practice.
- Overseeing and managing staff responsible for patient care coordination and management, which includes Health and Wellness Advisors Levels I and II.
- Conducting monthly audits of teams and sharing findings during supervision to promote accuracy and compliance.
- Problem-solving and addressing care issues requiring escalation in a manner that fosters member satisfaction and promotes quality care and service delivery.
Requirements
To succeed in this role, you will need:
- A Bachelor's degree from an accredited college or university in a healthcare-related field.
- Minimum of 5 years of Case Management experience.
- Minimum of 3 years of managerial experience and leadership in a Case Management role within a managed care organization or hospital setting.
- Knowledge of NCQA's Case Management Accreditation Standards.
- Proficiency in Microsoft Office and strong data analytical skills.
