We are currently seeking a full-time Patient Navigator to enhance our operations at the Southampton location.
Role Summary:
The Patient Navigator plays a crucial role in identifying individuals and their families within the target demographic who require preventive and primary healthcare services, facilitating their access to necessary resources. This includes support for:
- Acute, chronic, and communicable diseases
- Dental care
- Prenatal services and family planning
- Nutrition programs such as WIC
- Pediatric healthcare and immunizations
Key Responsibilities:
- Collaborate with medical personnel to connect patients with services, providing support such as translation, transportation, and application assistance.
- Coordinate patient appointments across various departments and external agencies, adhering to established referral procedures.
- Assist patients in overcoming barriers to accessing required referrals.
- Guide patients to appropriate agencies, focusing on specialty care and public health insurance programs, while tracking progress in the electronic health record system.
- Address patient inquiries to clarify provider instructions and referral requirements.
- Ensure all necessary clinical information is transmitted to specialists for referrals and obtain consent for information release.
- Review pending referrals for upcoming medical appointments and communicate with the clinical team.
High School Diploma or GED is required, along with 2-4 years of relevant experience.
Compensation:
Salary is set at $22.00 per hour.
