Job Description:

As a Case Management Specialist with Health New England, you will play a vital role in ensuring that our members receive high-quality, coordinated care. In this dynamic role, you will work collaboratively with our interdisciplinary team to assess patient needs, develop personalized care plans, and provide education and support to promote healthy behaviors and disease management.

Your expertise in clinical assessment and decision-making will enable you to identify and address barriers to care, while your exceptional communication skills will facilitate collaboration with internal and external partners to achieve optimal outcomes. You will also have the opportunity to contribute to the development and implementation of new programs and initiatives aimed at improving healthcare delivery and patient outcomes.

The ideal candidate will possess a strong educational background in nursing or a related field, combined with significant experience in case management, utilization management, or healthcare navigation. A Bachelor's degree is required, with a Master's degree preferred. Certification in case management (CCM) is also a plus.

**Key Responsibilities:**
  • Assess patient needs and develop individualized care plans to promote health and well-being
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination and transition
  • Identify and address barriers to care, using evidence-based practices to improve health outcomes
  • Maintain accurate and timely documentation of patient interactions and care planning activities


Requirements:
  • Bachelor's degree in nursing or related field; Master's degree preferred
  • Current licensure as a Registered Nurse (RN); CCM certification a plus
  • 3-5 years of experience in case management, utilization management, or healthcare navigation; experience in managed care, home care, or hospital settings an asset
  • Excellent clinical assessment and decision-making skills, with ability to navigate complex healthcare systems
  • Strong analytical and problem-solving skills, with ability to identify and address barriers to care
  • Exceptional communication and interpersonal skills, with ability to collaborate with diverse stakeholders


Estimated Salary Range: $63,000 - $83,000 per year, based on experience and qualifications. We offer a comprehensive benefits package, including health insurance, paid time off, and opportunities for professional growth and development.
