Support and Empowerment Specialist

We are seeking a highly skilled Support and Empowerment Specialist to join our team at Pathlight Group.
This is an excellent opportunity for a dedicated professional who is passionate about supporting individuals with developmental and intellectual disabilities.
About the RoleThe ideal candidate will have a strong background in human services or social work, with experience in case management and supervision. They will be responsible for providing support and guidance to individuals with developmental disabilities, as well as coordinating services with care providers, foster parents, and other stakeholders.
Key Responsibilities:
- Provide case management services to individuals with developmental disabilities, including assessments, goal setting, and service planning.
- Coordinate services with care providers, foster parents, and other stakeholders to ensure continuity of care.
- Supervise direct support staff and care/respite providers to ensure high-quality services.
- Work collaboratively with external agencies, including the Department of Developmental Services, Department of Children and Families, and MassHealth.
- Prepare reports and assessments as required, including crisis intervention services and on-call support.

What We OfferIn addition to a competitive salary range of $48,380.80 - $52,353.60 per year, we offer a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.
RequirementsTo be successful in this role, you will need:
- A degree in human services or social work, or related field.
- Over 3 years of experience in case management and supervision, preferably in a similar role.
- Strong knowledge of computer systems and applications, including databases and reporting tools.
- A valid driver's license and reliable transportation.
