About the Position: Nurse - Behavioral Health Services

LHC Group is committed to providing high-quality, compassionate care to patients in their homes. We are seeking a skilled and experienced Registered Nurse to join our team as a Nurse - Behavioral Health Services.

**Job Summary:**
The successful candidate will be responsible for providing behavioral health and other community-based healthcare services to patients in Hartford and surrounding areas. This role requires strong communication skills, both verbal and written, as well as the ability to work independently and as part of a multidisciplinary team.

**Responsibilities:**
This role involves providing high-quality clinical services, completing clinical nursing assessments, and developing and revising individualized plans of care. The successful candidate will work closely with other healthcare professionals to ensure plan of care frequency and duration meets the needs of the patient.

**Education and Experience:**
To be considered for this position, applicants must have a minimum of one year experience as a Registered Nurse and current RN licensure in the state of practice. A valid driver's license and access to a reliable vehicle are also required.

**Salary:** $95,000 - $130,000 per annum

**Location:** Hartford and surrounding areas (Farmington, Avon, Bristol, and Southington)
