Job Summary

Oak Street Health is seeking a motivated and compassionate Healthcare Advocate to join our team. As a Healthcare Advocate, you will play a critical role in bridging the gap between our patients, community, and medical systems. Your primary responsibility will be to act as a patient advocate, accompanying patients through proactive in-person and phone outreach based on their care needs. This will involve promoting health literacy, increasing access to resources, and removing barriers to wellness across all life domains.

Key Responsibilities

  • Establish and maintain strong interpersonal relationships with patients, community organizations, and team members to coordinate patient needs.
  • Manage patient referrals and collaborate with the Medical Social Worker on action plans.
  • Facilitate communication between patients, family members, caregivers, medical providers, and community-based organizations as needed.
  • Build relationships with local community organizations and connect patients to state and local community resources related to housing, transportation, food, and activities of daily living.
  • Assist patients with completing applications for accessing eligible benefits and resources.
  • Promote goal setting and achievement to improve patients' quality of life and self-efficacy.

Requirements

  • Minimum of 1 year of experience in healthcare, community-based, case management, or social service environment.
  • Strong oral and written communication skills.
  • Ability to manage multiple priorities while maintaining a positive attitude.
  • Dedication to serving the community and building meaningful relationships.
  • Proficient computer skills and access to reliable transportation.

Preferred Qualifications

  • Fluency in a language commonly spoken in the community (e.g., Spanish).
  • Experience working on multidisciplinary teams with organizations, agencies, patients, and community members.
  • Knowledge of community resources and resource navigation.
