Job Summary

We are seeking a highly motivated and compassionate Care Specialist to join our team. As a Care Specialist, you will play a vital role in delivering chronic care management to high-complexity patients in their homes and communities. Your primary responsibility will be to facilitate virtual visits, conduct needs assessments, and coordinate care between patients and our interdisciplinary team.

Key Responsibilities:
  • Conduct direct outreach to patients via phone, in-person, and other strategies to enroll them in our program and collect key data.
  • Meet patients in their homes or communities to conduct needs assessments, set health goals, and facilitate in-home provider appointments.
  • Support patients in meeting their healthcare goals, working within an interdisciplinary team to meet market and organizational goals.
  • Analyze patient data to determine patient needs or treatment goals, and assess physical conditions to aid in diagnosis and treatment.
  • Explain technical medical information to educate patients, and cultivate relationships with primary care providers to ensure seamless care coordination.
Requirements:
  • At least 2 years of relevant work experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role.
  • High school graduate or GED required, with a valid driver's license and auto liability insurance.
  • Reliable transportation and ability to travel within assigned territory or other areas as needed.
  • Experience in Chronic Care Management or working with chronically ill/elderly patients is preferred.
  • Ability to complete Upward Health's initial training program and ongoing educational requirements.
