Rehab Liaison RN

We are seeking a skilled Rehab Liaison RN to join our team at Providence Home Services Oregon. As a key member of our healthcare team, you will provide direct nursing services related to infusion therapy for patients in various settings, including hospitals, homes, clinics, and alternate living sites.
Key Responsibilities- Provide education to patients discharging from area hospitals, focusing on infusion therapy and related care.
- Collaborate with various providers, patients, caregivers, physicians, and payors to ensure seamless care coordination.
- Deliver infusion nursing care in the home or clinic, prioritizing patient safety and satisfaction.
- Develop and implement individualized care plans, incorporating patient goals and preferences.
- Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care.
- Licensure in states where care is delivered upon hire.
- 1 year of experience in acute hospital care, infusion, or home health.
- Strong communication and interpersonal skills, with a focus on patient-centered care.
- Ability to work independently and as part of a team, prioritizing patient needs.
