Job Summary

We are seeking a skilled Rehab Liaison RN to provide direct nursing services related to infusion therapy for patients in various settings. As a key member of our team, you will be responsible for providing education to patients discharging from area hospitals, as well as providing infusion nursing care in the home or clinic.

Key Responsibilities
  • Provide direct nursing services related to infusion therapy for patients in various settings
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with various providers, patients, and caregivers to ensure seamless care coordination
  • Provide education to patients and caregivers on infusion therapy and related topics
  • Conduct regular assessments and evaluations to ensure optimal patient outcomes
Requirements
  • Current RN licensure in states where care is delivered
  • 1 year of experience with acute hospital care, infusion, or home health
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
