Job Summary

We are seeking a skilled Family Case Manager to join our team at Berkshire Farm Center. The successful candidate will be responsible for coordinating comprehensive case management plans for youth and families to ensure the safety, permanency, and well-being of youth in foster care.

Key Responsibilities
  • Conduct assessments of family strengths and needs, and develop treatment plans to address problem areas.
  • Participate in the development, implementation, and evaluation of comprehensive treatment and discharge plans.
  • Develop alternative placement or permanency resources for youth who may not return to their families.
  • Deliver effective case management services, including scheduling counseling sessions and ensuring children receive required medical and dental examinations.
  • Track and monitor services provided, and complete all documentation in a timely and comprehensive manner.
Requirements
  • Minimum Bachelor's degree in human services or a related field.
  • Minimum one year of successful human service, social services, child welfare, case management, mental health, foster care, or related field experience.
  • Ability to work a flexible schedule with variable hours, including day, evening, weekend, and holiday hours.
  • Valid driver's license and reliable vehicle.
