Role Summary

Bear Mountain Hanover Terrace Health & Rehabilitation Center seeks a skilled RN Infection Preventionist to lead our infection prevention and control program. The successful candidate will be responsible for developing and implementing a comprehensive program to prevent the development and transmission of communicable diseases and infections.

Key Responsibilities
  • Develop and oversee the facility's infection prevention and control program, ensuring compliance with regulatory guidelines.
  • Collaborate with facility leadership and the medical director to establish annual infection prevention goals and action plans.
  • Partner with facility leaders, physicians, and external agencies to promote infection prevention practices.
  • Establish a facility-wide system for identifying, investigating, and controlling infections.
  • Conduct outbreak investigations and maintain a system for reporting incidents to relevant authorities.
  • Oversee the antibiotic stewardship program and monitor residents' antibiotic use.
  • Develop and present IPCP in-service education for staff.
  • Maintain current knowledge of state, federal, and local guidelines and regulations related to infection prevention and communicable diseases.
Requirements
  • Current and active Registered Nurse licensure in N.H.
  • Previous experience in Skilled Nursing Facility or other Long Term Care Setting preferred.
  • Certification in Infection Prevention (CDC) or willingness to complete educational requirements.
Work Schedule

Primary hours of position: Mon-Fri 8am-4:30 pm with some flexibility in daily hours available. Some weekend or off-hour flexibility may be required dependent on facility needs.
