Job Title: Health Equity Patient Advocate

Overview: The Erie County Medical Center Corporation (ECMCC) is seeking a highly motivated and detail-oriented individual to fill the role of Health Equity Patient Advocate. This position plays a vital role in facilitating continued patient care by eliminating barriers to care and providing outreach for patients at ECMCC.

Responsibilities:

  • Develops statistics and patient data related to health equity for inclusion in records, reports, and other documentation utilizing computerized and electronic databases;
  • Serves as a liaison for patients and/or caregivers with various facilities to obtain test results;
  • Receives patient medical information and inputs into an electronic health records system;
  • Assists with conducting comprehensive assessments of patients' social, economic, and cultural needs and utilizes assessment to develop and implement individualized care plans that address barriers to health equity;
  • Monitors and tracks patient progress, ensuring adherence to care plans and making adjustments as necessary;
  • Identifies and connects patients with appropriate community resources and support services;
  • Maintains and monitors electronic health records to identify patients by risk factor, those requiring follow-up appointments, and those not meeting treatment goals;

Requirements: To be successful in this role, you will need strong communication skills, attention to detail, and the ability to work independently. A high school diploma or equivalent is required, along with 2 years of office clerical experience, including 1 year in a healthcare setting. Verifiable part-time and/or volunteer experience will be pro-rated toward meeting full-time experience requirements.

Salary: $25-$32 per hour
