Compensation Package

We offer a competitive hourly rate of $32.50 - $42.50 per hour, based on a full-time, 40-hour work week, considering various factors unique to each candidate.

Job Responsibilities

  • Build Trusting Relationships: Facilitate access to healthcare services and meet patients' health needs by building trusting relationships.
  • Engage Patients in Health Promotion: Encourage patients to participate in health promotion, maintenance, and restoration activities.
  • Work Collaboratively: Partner with our multidisciplinary team to advocate for patients and ensure their health needs are met.
  • Assist Patients and Families: Help patients and families navigate internal providers, community resources, and other services as needed.
  • Conduct Community Outreach: Identify people experiencing homelessness and educate them about health needs through community outreach, home visits, shelter/street outreach.

Requirements and Qualifications

  • Nursing Degree: Hold a graduated degree from an accredited two-year nursing school.
  • Licensure and Certification: Possess a current NYS Professional Registered Nurse license and CPR certification.
  • Clinical Experience: Have knowledge of case/care management principles and experience working with diverse patient groups.
  • Skill Set: Demonstrate exceptional attention to detail, superb organizational skills, strong influencing skills, and self-motivation.
