Reports To: Youth Empowerment & Violence Prevention Manager

Classification: Full Time, 1.0 FTE

FLSA Status: Non-Exempt

Compensation: $25.96 per hour

Role Responsibilities

The Family Care Program Manager will support the Youth Empowerment and Violence Prevention Manager, as well as the CHI Adult Mentor, in achieving program objectives through the Community Initiative Healing model. This role involves case management for Latino males aged 18-30 who may have a history of gang involvement, criminal offenses, or are at high risk of re-offending. The Manager will collaborate with clients, their families, and various service providers to ensure successful outcomes and access to necessary services.

Key Responsibilities Include:
  • Collaborating with the Program Manager and CHI Elevate Mentor to align with the La Cultura Cura: Joven Noble Curriculum.
  • Assisting in the implementation of the La Cultura Cura curriculum.
  • Applying the Community Healing Initiative Model in client interactions.
  • Developing strength-based individual and family success plans based on culturally relevant and evidence-based practices.
  • Understanding and valuing Multnomah County Assertive Engagement goals and participating in interdisciplinary team settings.
  • Effectively collaborating with partner agencies and community justice staff.
  • Conducting Intake Assessments to identify client needs and strengths.
  • Providing services in diverse settings beyond traditional office environments.
  • Attending case staffing meetings and training sessions.
  • Working closely with probation and parole officers to monitor client progress.
  • Overseeing the implementation of comprehensive service plans for clients and their families.
  • Securing crisis intervention services as necessary.
  • Offering support in areas such as identity development, health, employment, education, and life skills.
  • Maintaining detailed case files in compliance with legal and ethical standards.
  • Documenting and submitting program expenditures promptly.
  • Fostering a collaborative team environment.
  • Ensuring regular and predictable attendance.
  • Respectfully following supervisory direction.
  • Performing other duties as assigned.
Qualifications
  • Experience in social work with families in the target demographic, or a combination of education and relevant experience.
  • Bilingual proficiency in Spanish and English.
  • A minimum of two years' experience in case management.
  • Understanding of issues related to substance abuse, mental health, and adolescent development.
  • Preferred experience working with high-risk Latino youth and families.
  • Commitment to working with diverse populations.
  • Ability to conduct presentations in both English and Spanish.
  • Strong communication and interpersonal skills.
  • Knowledge of community resources and social services in the Portland Metro area.
  • Familiarity with culturally based perspectives and problem-solving practices.
  • Experience with Latino youth and families.
  • Willingness to work in a multi-disciplinary team environment.
Additional Requirements
  • Availability to work in both office and community settings.
  • Evening and weekend availability may be necessary.
  • Reliable transportation and valid driver's license.
  • Completion of criminal background checks is mandatory.

The role may require the ability to perform essential functions, including lifting up to 50 pounds and engaging in various physical activities.
