Job Summary

Element Care is seeking a skilled and compassionate Enrollment Intake Nurse to join our team. As a key member of our enrollment team, you will play a vital role in prioritizing, organizing, and adapting the PACE enrollment process to meet the individualized needs of potential participants.

Key Responsibilities
  • Initial Appointments: Conduct initial appointments to describe the PACE model to potential participants and their caregivers, ensuring they understand the program's benefits and requirements.
  • Eligibility Evaluation: Verify that potential participants meet eligibility criteria in accordance with PACE enrollment regulations, ensuring a smooth and efficient enrollment process.
  • Needs Assessment: Utilize a strength-based approach to evaluate the current needs and health status of potential participants, providing valuable insights to inform the enrollment process.
  • Communication and Collaboration: Proactively communicate with the Interdisciplinary Team (IDT) regarding current enrollment status and progress of prospective participants, ensuring seamless coordination and collaboration.
  • Mediation and Prioritization: Mediate and prioritize between prospective participants'/caregivers' needs and the IDT's concerns, ensuring that the enrollment process is tailored to meet the unique needs of each individual.
  • Enrollment Team Meetings: Participate in weekly Enrollment Team meetings to provide clinical insights, advice, and additional support regarding challenging referrals and other barriers that may arise.
  • Data Management: Comply and maintain accurate data on new referrals in the CRM, EMR software, and Masshealth Virtual Gateway, ensuring data integrity and informed decision-making.
  • Marketing and Outreach: Coordinate and assist Marketing and Outreach with outside events, promoting the PACE program and its benefits to potential participants.
Requirements
  • Education: Associate's or Bachelor's degree in Nursing, or completion of a Nursing Diploma Program.
  • Licensure: Licensed as a Registered Nurse in the Commonwealth of Massachusetts.
  • Experience: Minimum one (1) year experience in a geriatric healthcare and/or adult mental health setting.
  • Skills: Advanced computer skills, including Microsoft Office, Customer Relationship Management, and Electronic Medical Records software. Ability to communicate effectively with and establish working relationships with various medical licensed personnel.
  • Qualifications: Strong knowledge of medical/psychiatric diagnosis and treatments. Experience working competently and comfortably with individuals from diverse socio-cultural backgrounds. Ability to assess multiple perspectives and priorities and successfully facilitate within a goal-oriented/solution-focused framework.
