Public Health Management Corporation is proud to be a leader in public health, promoting the wellbeing of our community. We are currently seeking a skilled Nurse-Family Partnership Public Health Nurse Home Visitor to join our team.
Job Overview:
- This role involves providing comprehensive nursing services to first-time pregnant individuals and their families eligible for the NFP Program in Philadelphia.
- The successful candidate will maintain the highest standards in clinical practice, adhering to the national Nurse-Family Partnership model.
- The program focuses on supporting families to meet their life goals, with a goal that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken.
Responsibilities:
- The Nurse-Family Partnership Public Health Nurse Home Visitor will complete all required NFP and NNCC education, including training in the Strengths and Risks (STAR) Framework, Facilitating Attuned Interactions (FAN), Ages and Stages Developmental and Social Emotional Assessments, Partners in Parenting Education (PIPE), Mental Health and Intimate Partner Violence trainings, Cultural Consciousness training, Dyadic Assessment of Naturalistic Caregiver-Child Experiences (DANCE), and regional/state education sessions.
- They will provide regularly scheduled in-home visits and telehealth visits with clients, building and maintaining a caseload of 25 active clients within nine months of employment.
- The successful candidate will develop therapeutic relationships with clients utilizing concepts and processes from motivational interviewing, reflective listing, and FAN.
- They will plan home visits in accordance with client goals and NFP outcomes, supporting the achievement of client-identified goals through one-on-one education, active listening, screening, and referral processes to community resources.
- The Nurse-Family Partnership Public Health Nurse Home Visitor will document all work appropriately and in a timely fashion using the Apricot and Penelope (Flo) Case Management Platforms.
- They will uphold the policies, procedures, guidelines, and standards of NFP and NNCC, adhering to American Nurses Association and Pennsylvania nursing process and standards, code of ethics, HIPAA confidentiality standards, and the NFP model of home visitation.
- The successful candidate will contribute to and participate in virtual and on-site clinical case conferences and professional development meetings.
- They will meet weekly with the NFP supervisor for clinical reflective supervision and utilize the reflective process in supervisory sessions.
- The Nurse-Family Partnership Public Health Nurse Home Visitor will utilize nursing process to assess, plan, and document physical, emotional, social, and environmental needs of clients and their families as they relate to the NFP domains of care.
- They will schedule joint home visits with NFP Supervisor every four months (three times a year) to support professional development.
- The successful candidate will consult and collaborate with other professionals involved in providing services to clients and their families.
- They will collaborate with NNCC outreach staff regarding community sources and disposition of referrals to NFP program.
- The Nurse-Family Partnership Public Health Nurse Home Visitor will contribute to a positive work environment that enables all staff to function at their highest potential.
- They will provide timely documentation to data entry staff for input.
- The successful candidate will review documentation and data system reports entered in state, local, and national databases for accuracy, uniformity, and completeness.
- They will demonstrate a commitment to serving the greater good of the Philadelphia community and appreciation of public health implications.
- The Nurse-Family Partnership Public Health Nurse Home Visitor will recognize the role of race, income, age, immigration status, and other individualities in shaping health inequities and dependably advocate for more just policy results.

- The Nurse-Family Partnership Public Health Nurse Home Visitor will be comfortable creating structured environment for self and able to adapt a flexible schedule of home visiting.
- They will possess professional speaking and writing skills that are responsive to the audience served.
- The successful candidate will have strong documentation skills; documentation is entirely computerized.
- They will be detail-oriented, able to focus on task at hand, and to catch errors and fix them efficiently and professionally.
- The Nurse-Family Partnership Public Health Nurse Home Visitor will have moderate to advanced computer skills, able to use Outlook and Microsoft Teams.
- They will be able to work collaboratively with professionals, families, and community members, viewing the client as the expert on their own life.
- The successful candidate will be capable of building and maintaining relationships with a diverse group of clients over a long-term period.
- They will possess cultural humility and respect for differences among clients and peers.
- The Nurse-Family Partnership Public Health Nurse Home Visitor will be comfortable entering and providing home visits in unfamiliar spaces.
- They will actively work to understand and address their personal reactions to clients, welcoming feedback non-defensively, and make good use of the reflective supervision provided.
- The successful candidate will have a growth mindset about people, believing that individuals can learn, change, grow, and improve.
Requirements:
- Current Pennsylvania nursing licensure in good standing.
- Work or student experience in public health, maternal-child health, mental health/behavioral health, or related field such as Early Childhood services in community settings.
Estimated Salary: $60,000 - $80,000 per year based on qualifications and location.
