We are seeking a compassionate and motivated Public Health Nurse Home Visitor to join our team at Public Health Management Corporation. As a key member of our organization, you will play a vital role in providing comprehensive community nursing services to pregnant individuals and/or their families eligible for the Mabel Morris Family Home Visit Program.

About the Role

The Public Health Nurse Home Visitor (Mabel Morris Program) is responsible for delivering high-quality home visits and telehealth services to clients while maintaining the highest standards in clinical practice. You will work collaboratively with families to implement evidence-based strategies connecting resources needed to meet goals for their children's and family's holistic health and educational well-being.

  • Deliver comprehensive community nursing services to pregnant individuals and/or their families.
  • Maintain the highest standards in clinical practice and adhere to the Parents as Teachers (PAT) model.
  • Build and maintain a caseload of 18-20 active families raising children under five years old within nine months of employment.
  • Develop therapeutic relationships with clients utilizing concepts and processes from motivational interviewing, reflective listing, and program strategies.

Responsibilities

Your key responsibilities will include:

  • Providing regularly scheduled in-home visits and telehealth visits for clients.
  • Completing all required PAT and National Nurse-Led Care Consortium (NNCC) education.
  • Supporting the achievement of client-identified goals through one-on-one education, active listening, screening, and referral processes to community resources.
  • Conducting joint home visits with the PAT Supervisor every six months.
  • Collaborating with outreach staff regarding community referral sources and disposition of referrals to the MM program.
Requirements

To be successful in this role, you will need:

  • A Baccalaureate Degree in Nursing from an accredited university.
  • Current Pennsylvania nursing licensure in good standing.
  • Work or student experience in public health, prenatal-child health, mental health/behavioral health, or related field.
  • Flexible schedule including 2-3 early evenings every week required and option to work Saturdays.
