Career Opportunity

We are seeking a skilled Care Coordinator to join our team. This role is responsible for assisting patients with their needs, managing care, and addressing social determinants of health for Medicaid recipients with chronic health conditions.

Responsibilities
  • Build and maintain relationships with patients, providing them with personalized support and care.
  • Conduct face-to-face assessments to identify medical and social needs, creating effective care plans in collaboration with providers and caregivers.
  • Provide community resources to patients, promoting health and well-being through education and referrals.
  • Schedule appointments and transportation, ensuring timely access to appropriate care.
Qualifications
  • At least 2 years of experience in social services.
  • An Associate's degree or equivalent required.
  • Bilingual Spanish skills are strongly preferred.
Benefits
